If you have ever spotted white spots or bumps on your testicles, you
are not alone. Several medical conditions can cause these spots and
the good news is --most of the conditions are fairly easy to cure. Are
the white spots contagious? What is causing these spots and will they
go away? White testicular spots can be worrying and may even be
irritating. You’re probably also worried what your sexual partner will
think of those white bumps.
Don’t panic. Put those catastrophic thoughts of cancer out of your
head. White spots on the testicles or penis are not usually the sign of
anything serious, and are hardly ever the sign of cancer of the penis or
testicle. And the chances are a sex partner will never notice the small
white bumps. Take a closer look at what is causing the spots, and what
you need to do to get rid of them.
What are the Most Common Causes of White Spots on the Testicles?
Just as you can get pimples on your face, you can also get them on
your scrotum or penis. Sometimes a white spot or spots occur on the
Alternatively, the white spots are often prominent hair follicles – most
men have a few hairs growing around the base of the penis and the
The tiny pit out of which they grow is called the follicle and it can often
be quite visible and white. In both these cases, there is nothing to
worry about and no treatment is needed. Don’t squeeze these spots,
however, as you could cause an infection.
Other causes of white spots on the testicles include inflammatory skin
conditions, sexually transmitted diseases, allergies, or autoimmune
Should I See a Doctor About the White Spots on my Testicles?
If you are at all concerned about white spots on your testicles or penis,
you should see a doctor – go to your regular physician or see a
specialist at a sexual health clinic. Don’t be embarrassed about visiting a
doctor with this problem. Medics have seen it all before and will be
sympathetic to your condition.
We looked at the many different reasons for white spots on the
testicles, backed by scientific research and collected them here for you:
1. White Spots on the Testicles May Be Caused By Lichen Sclerosus
Lichen sclerosus is a chronic inflammatory disease that shows itself as
white patches on the penis, foreskin, and around the genital area,
according to a 2002 study from the University of Basel, Switzerland.
Small white spots are an early sign of the disease, which can progress
to larger patches, crinkled skin, and bright red or purple bruises.
No one really knows why lichen sclerosus happens although doctors
believe that a too-active immune system or hormone issues may play a
part. If you have lichen sclerosus on the skin of the genitals it should
be treated as it may cause scarring.
Treatment may involve strong cortisone cream or retinoids according to
the National Institute of Arthritis and Musculoskeletal and Skin Diseases
2. They May Be Fordyce Spots – No Treatment Needed?
Fordyce spots are tiny white or yellow-tinged spots found on the shaft
of the penis or the head. They may also be present on the testicles.
spots are normal and a natural part of the genital structure, according
to a 2013 study by Melbourne Sexual Health Centre, Australia, and
many doctors advise against treatment.
However, for men concerned with the appearance of these spots, CO2
lasers can be used, as can a micro-punch technique which
demonstrated success in a 2013 study from University Hospital RWTH
3. Lichen Planus Causes White Spots on the Testicles
Lichen planus can occur as raised white spots on several parts of the
body including the testicles and genital area. While doctors are not sure
of the exact causes, lichen planus may be an autoimmune disease.
Bad news, there is no cure for the condition. But good news, it often
goes away on its own. If your lichens planus persists, consider using
antihistamines and topical corticosteroid cream to reduce the itching,
PUVA light therapy, or retinoic acid may be prescribed, according to the
American Academy of Dermatology.
4. White Spots on the Testicles May Be Genital Warts
While many of the causes of white spots on the testicles do not need
treatment, there is one condition that does – genital warts.
Genital warts are pink, ivory, cream or brown colored depending on
your skin tone. They are sexually transmitted and are caused by one of
the strains of the human papilloma virus (HPV). Treatment is through
the application of creams and paints, or by removing the wart
altogether with a laser or electric probe.
The Centers for Disease Control and Prevention say that half of all
sexually active Americans will get HPV at some point in their lives. A
2011 study from the H. Lee Moffitt Cancer Center and University of
California - San Francisco demonstrates that the vaccine for human
papillomavirus (HPV) can prevent 90 percent of genital warts in men,
when the vaccine is given before exposure to the strains of HPV
covered by the vaccine.
5. Allergic Contact Dermatitis Can Cause White Genital Spots
Allergic contact dermatitis can occur anywhere on the body but when it
occurs on the testicles or in the genital area it is most likely caused by a
reaction to “condoms, lubricants, feminine hygiene deodorant spray
and spermicides” according to a 2002 study from the University of
In this type of dermatitis the skin is injured by exposure to chemicals
and the condition can result in white patches or spots on the skin. The
British Association of Dermatologists says emollient creams, topical
steroids, and antibiotics for any secondary infection can help assist in
6. Pearly Penile Papules Look Like White Spots on the Testicles
Pearly penile papules are noninfectious penile lesions, as detailed by
researchers at the University of British Columbia, Canada in 2010.
These very small bumps are normally the same color as the skin or
slightly whiter with a pearly appearance, and they are normal. These
bumps are not sexually transmitted and they do not need to be
removed although you can do so for cosmetic reasons.
A 2015 study from the University of Michigan, Ann Arbor shows that
penis papules can be successfully treated with fractionated CO2 laser.
7. Could White Spots on the Genitals Be Cancer?
White spots on the testicles or on the penis are hardly ever a sign of
cancer of the penis or testicular cancer. Both cancers are rare. The
American Cancer Society says that in 2014 there were around 1,600
new cases of penile cancer and the rate of testicular cancer in the US is
5.1 per 100,000 according to Cancer Research UK.
Symptoms of penis or testicular cancer most likely include a lump in the
penis or the scrotum, and not white spots.
